Endoscopic management of large ileal hamartomatous polyps in Peutz–Jeghers syndrome using “Triple-Balloon Enteroscopy” with Endorail® system

Case Report

A patient with known Peutz-Jeghers syndrome previously underwent videocapsule endoscopy revealing multiple small-bowel polyps. Anterograde double-balloon enteroscopy (DBE) allowed removal of a jejunal sessile hamartomatous polyp. Retrograde-DBE identified a 3–4 cm pedunculated ileal hamartomatous polyp. A further retrograde-DBE using the recently CE-marked Endorail® system provided improved scope stability, enabling successful endoloop placement. The Endorail® device uses a magnetic balloon that can be advanced beyond the enteroscope tip, filled with a ferromagnetic fluid, and magnetically anchored to the patient’s abdomen, allowing it to be straightened and guided along the device. Endorail® can be used both to overcome challenging bowel loops and increase insertion depth, as well as to provide greater procedural stability for effective endoscopic therapy.
